Targeted Case Manager Coordinator

The TCM Coordinator will report directly to the Director of Programs in all areas of job performance and responsibilities.

The TCM Coordinator shall be at least 21 years-old and will have a bachelor’s degree in social work or other human service field and one year of post graduate experience working directly with the targeted population or performing case management services.   

Must have a current driver license and must have completed a 12-hour Core TCM training while meeting all continuing education requirements.

As an employee at DCCH Center for Children and Families, the Case Manager shall be sensitive and responsive to cultural differences, which are present in the organization’s service population. All employees will participate in supporting a culture that is Trauma Informed. 

As an employee of DCCH Center, the TCM will participate in systematic performance improvement efforts and will support and promote the PQI (Performance Quality Improvement) program throughout the agency.

Principle Responsibilites

  1. Manage a caseload of up to 22 children.
  2. Assist clients in accessing behavioral health treatment, medical interventions, and educational, social, and other support services as needed. 
  3. Coordinate services, develop treatment plans, and make referrals as necessary to meet the needs of the children. 
  4. Attend all State mandated training to be able to provide services.
  5. Communicate with the family, providers, and agency partners regarding progress in care.
  6. Maintain and monitor all required documentation in clients records according to timelines.
  7. Conduct home visits and meet State requirements for contact and documentation.
  8. Work with MCOs to understand each one’s authorization and documentation requirements. 
  9. Coordinate program functions such as billing as well as developing and overseeing the program.
  10. Meet all OIG and COA regulations. Update policies and procedures and a training Manuel. 
  11. Market the program on-line and through face-to-face contacts.
  12. Hire, train, support, and supervise any agency TCMs.  
  13. Perform other duties as assigned by the Director of Programs or the Executive Director.
