Our goal
Our goal is to provide support to children and adults by offering access to resources. Ultimately, we want clients to be able to thrive within their family setting and avoid separations. We will assist clients in obtaining and maintaining connections within their community to improve their overall functioning.
Treatment flow
Certified case managers first help assess your needs and develop an individualized care plan. Our staff works closely with you and community providers to assist in eliminating barriers to and connecting you to resources. To help you achieve your goals and monitor your progress, we meet with you several times every month throughout the duration of your care plan.
Who is eligible?
Those who have Medicaid insurance and have a qualifying mental health issue. Also, those in need of additional educational, emotional, social, housing and/or job services. You must be between the ages of 4 and 64.
Where does treatment take place?
DCCH case managers will meet you wherever is most convenient and comfortable for you—at our offices in Fort Mitchell, Kentucky, at your home, or at a community location where you feel safe
Counties we serve
DCCH Center for Children and Families serves individuals who live in Kentucky counties Boone, Campbell, Kenton, Grant, and Pendleton.
How long is treatment?
The length of treatment depends on one’s needs towards achieving goals, which can vary in length of engagement. The average length of treatment is 3-6 months. Our ultimate goal is to provide support for as long as services are needed.
